WILMINGTON, NC, May 11, 2018 — Well Care Health unveiled today “Total Patient Management,” the company’s population health and clinical integration framework. The program’s purpose is to deliver comprehensive patient care, with the goal of positioning Well Care as the leader of in-home population health care delivery, experience, and innovation.

This brand gives our clinical partners a clear explanation of Well Care’s approach to in-home patient care and reaffirms our commitment to addressing the entirety of patients’ needs, from wellness to end of life care,” Kipple added.
