WILMINGTON, NC, May 11, 2018 — Well Care Health unveiled today “Total Patient Management,” the company’s population health and clinical integration framework. The program’s purpose is to deliver comprehensive patient care, with the goal of positioning Well Care as the leader of in-home population health care delivery, experience, and innovation.
“Defining Well Care’s population health program has been a top priority for the organization,” said Robin Kipple, RN, Senior Director of Population Health and Clinical Integration. “Total Patient Management is our stake in the ground. This brand gives our clinical partners a clear explanation of Well Care’s approach to in-home patient care and reaffirms our commitment to addressing the entirety of patients’ needs, from wellness to end of life care,” Kipple added.

Well Care Health’s additional priorities include establishing partnerships with other post-acute care providers to give patients resources outside of the agency’s scope, and transition them to the appropriate professionals at the right time in their care cycle.
