RALEIGH, NC ― Well Care Home Health announces today that it has earned the Home Health Heart Failure certification from the American Heart Association® and is the first certified home health program of this kind in North Carolina.
The American Heart Association is the world’s leading nonprofit organization focused on heart and brain health for all. The certification provides an evidence-based framework for evaluating home health agencies against the American Heart Association’s rigorous science-based standards for heart failure patients, including program management, patient and caregiver education and support, care coordination, clinical management, and quality improvement.

Well Care For Your Heart, Well Care’s in-home Heart Failure Management program, is a comprehensive program driven by the American Heart Association’s clinical guidelines. The program includes education and interventions that focus on a heart-healthy diet, medication regimen, exercise, and self-monitoring. While participating in the program, patients can anticipate fewer hospitalizations, improved self-care, improved activity tolerance, and more. The program also features in-home chest x-rays, customized care plans, and interactive educational tools.
