The Well Care Hospice Team

At Well Care Hospice, patients and families are supported by a skilled, compassionate team that may include nurses, social workers, chaplains, hospice aides, physicians, and trained volunteers, all working together to provide thoughtful, coordinated care. We Remember

Our annual Remembrance Ceremony is a meaningful way to honor and remember the patients and loved ones who have touched our hearts. The butterfly release serves as a beautiful symbol of love, remembrance, and the lasting connection we carry forward. At Well Care Hospice, we believe these moments matter. Creating space for reflection and remembrance helps families feel supported, connected, and reminded that their loved ones will not be forgotten.

We Honor Veterans

At Well Care Hospice, honoring veterans is one of the many ways we bring deeper meaning to the work we do. Through the We Honor Veterans program, we recognize veteran patients with a special pinning and certificate presentation as a tribute to their service and sacrifice. These moments reflect our commitment to compassionate, personalized hospice care and to making sure every patient feels seen, valued, and honored.
